Once registered, you will be able to e-sign Medical Release Form and the e-sign Photo/Recording Release Form at http://stjosephssm.org under the Faith Formation Coordinator heading where you found this form that you are currently completing. Please print the Medical Release Form and Photo/Recording Release Form, complete, sign, and date form to be returned to St. Joseph’s Parish Office by September 20th. This will help us order books for classroom and family distribution.
